CBI Has The Capability To Function As Your Full Service SIU Department
SURVEILLANCE INVESTIGATION
- Surveillance of the Claimant is provided by CBI in order to document and videotape the Claimant's daily activities/abilities and determine if these activities/abilities are consistent with the claim or the Claimant's reported injury. All of the Claimant's activities, within the legal limits of state statutes, will be documented on videotape when possible.
PRE-SURVEILLANCE PROFILE
- A database report identifying a individual's name, date of birth, social security number, aliases, current address and telephone number (if available), possible relatives, neighbors, real property ownership, vehicle and vessel registration, bankruptcies, tax liens, judgments, UCC filings, possible criminal histories, parole and probation information, professional licensing, and possible corporate affiliations. This report is run either if 1) it is the first time that surveillance is being conducted on the claimant by CBI or 2) the last surveillance is over six months old.
LOCATE INVESTIGATION
- Using a wide array of nationwide database searches, along with possible in-person courthouse searches and interviews of previous neighbors, relatives, or acquaintances, we will assist you in locating hard to find claimants or witnesses.
ACTIVITIES CHECK INVESTIGATION
- Investigative attempt to ascertain useful information regarding the claimant (through legal means only) from local residents or neighbors of the claimant. The Activity Check service includes:
- Discreet contact of neighbors located in close proximity to the Claimant's residence to attempt to ascertain the Claimant's health, employment, physical limitations, and general activities at or away from the residence including lawn work performed, sporting activities involved in, and hobbies associated with the Claimant.
- A detailed description of the Claimant's residence, yard, neighborhood, important surroundings, and other pertinent facts. We will also obtain still photos of the Claimant's residence to document lawn condition and maintenance, construction projects, swimming pools, vehicles and boats, or any other information that may be an indicator of activity around the residence.
- A detailed assessment of the possible value of surveillance and the practicality of conducting surveillance on the Claimant's residence.
- Surveillance, in the event that the Claimant is observed outside the residence while conducting the Activity Check.
ALIVE AND WELL CHECK
involves an announced or unannounced (specify with service order) in-person interview with the Claimant to determine general health, current medical treatment and medications, next scheduled doctor's appointment, verification of employment and/or disability status, home environment, physical restrictions, hobbies and activities, description of a "day-in-the-life", social security number and date of birth verification, telephone number, spouse and children information, and photographs of the Claimant and their residence. This IS NOT done under pretext (i.e. the investigator will truthfully disclose intentions and whom he/she represents).
RECORDED OR WRITTEN STATEMENTS
- An interview that can either be recorded or written and signed from the Claimant regarding the facts pertaining to the Claimant's injury and also all information obtained from an Alive and Well Check above. Statements can be transcribed into a written report at the adjuster's request.
WITNESS STATEMENTS
- Interview recorded statement from all witnesses.
STATE DRIVER'S RECORDS
- A printed transcript of an individual's state motor vehicle driving record and offenses.
COMPREHENSIVE BACKGROUND
- A limited database background investigation in the current county of residence to include a Personal Profile Report, Vehicle and Vessel registration, 7-year Criminal and Civil history, Bankruptcies, Liens & Judgments, and state driving record.
COUNTY CRIMINAL CHECK
- A database background search of criminal records by county of residence only. Subject's name, date of birth, and social security number must be provided.
COUNTY CIVIL RECORDS CHECK
- A database background search of civil records by county of residence only. Subject's name, date of birth, and social security number must be provided.
FEDERAL CRIMINAL CHECK
- A database background search of federal criminal records by county of residence only. Subject's name, date of birth, and social security number must be provided.
LOCUS INVESTIGATION
- An investigation of an accident or incident scene with photographs, videotape, measurements, equipment layout, traffic and traffic control devices, signage, and any other circumstances pertaining to the accident or incident. The Scene Inspection Investigation can also be tailored to meet specific request(s).
